The Process: I have made filing your federal or state individual income tax return as easy and painless as possible. Use the checklist to the right to determine what you will need to bring to your appointment. Having the necessary documentation is a crucial factor in preparing an accurate return that takes advantage of as many tax credits that you are eligible for. The IRS Form 13614-C document is an interview questionnaire utilized by certified IRS volunteers. I use this form in my private income tax preparation practice because it offers one of the most effective methods to ensure income tax return accuracy. Together, we will review the completed form to determine your correct number of eligible dependents, filing status and other information critical to claim all eligible tax credits. After the interview process, I will electronically complete and file your return with the IRS. Simple returns get prepared while you wait. More complex returns may require up to 48 hours to ensure accuracy and IRS acceptance. You will receive a printed and/or electronic copy of your return sent to your email address along with a receipt for services. If you can expect a refund, I will also provide an estimated date of deposit. You will not need to mail a paper return. - Picture identification
- Social Security cards or other proof of Individual Tax Identification Number (ITIN) for all household members and dependents
- A copy of prior year's tax return if applicable
- W-2s, W-2Gs, and all other proof of income and wages from employers, gambling or lottery wins
- 1099-MISC forms for self-employment income
- 1099-INT (interest) and 1099-DIV (dividends) forms
- 1099-B forms showing brokerage trades in stocks and bonds
- K-1 forms for income from a partnership, small business, or trust
- 1099-SSA form showing Social Security and retirement benefits received
- Proof of other income received not reported elsewhere (pension, rental income, alimony, etc.)
- Proof of estimated tax payments
- Any other information regarding eligiblity for tax credits (proof of charitable gifts, property tax payments, educational costs, childcare costs including tax ID or social security number of childcare provider, IRA contributions, moving expenses, etc.)
- Bank routing and account numbers or credit card information for refund direct deposit or automated clearing house withdrawal of income tax payments due if applicable.
